Dear Participant,
This letter is a request for you to take part in a research project to assess unmet needs and barriers that exists for people with lived experience of a TBI. As a participant, you are being asked to participate in an online survey (or paper survey if you indicate you would rather participate on paper). The survey will ask a series of questions about you and your experiences with traumatic brain injury and services for people who have a brain injury.
This project is being conducted by Dr. Amanda Acord-Vira, Dr. Steven Wheeler, Dr. Heather Livengood, Cortland Nesley, BFA, the WVU CED TBI Program Staff, and OTD student Megan Bruce in the Division of Occupational Therapy at WVU to fulfill requirements for a Doctorate Degree in Occupational Therapy. Your participation in this project is greatly appreciated and will take approximately 15-20 minutes to fill out the following survey. Your involvement in this project will be kept as confidential as legally possible. All data will be reported in the aggregate. You must be 18 years of age or older to participate. We will not ask any information that could lead back to your identity as a participant. Your participation is completely voluntary. You may skip any question that you do not wish to answer and you may discontinue at any time. West Virginia University's Institutional Review Board acknowledgment of this project is on file.
